We invite prospective new patients to come down to the Health Centre to fill in a Registration Form (for patients over 16). One person can fill in a Patient Registration Form for each family member. You will also need to bring with you two forms of ID for all patients over the age of 16 (ID Requirements). If you are registering a new baby, please complete a Newborn Registration Form.
If you have a Long Term Condition such as Diabetes, Asthma, COPD etc. our registrations clerk will contact you to inform you of the services we offer in terms of your care.
For patients taking medication, once registered we ask that you make an appointment see a GP when your first prescription from us is due.

However there are exceptions to the above. Where a relative moves into the same house as an existing patient they will be accepted.
If an existing patient moves to a new home which is outside our practice boundary we will have to ask you to register with another practice. Due to the pressures of new housing leading to our list size increasing constantly, we are unable to make any exceptions to this rule.

A Patient Guide to GP Services
These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

If you are ill while away from home and need to see a doctor you can receive emergency treatment from the local GP practice by registering as a temporary resident.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
