Following recommendations from medical bodies regarding confidentiality our safeguarding and Caldicott lead has reviewed our policy on consent for patients aged 11 years and over.
Consent for access to online access to medical record information
Patients aged up to and including the age of 10 years
Proxy access for parents/guardians is allowed for **booking appointments, ordering medication and detailed coded records access.
Patients aged 11 and 12 years inclusive
From the age of 11 years old a patient can deny parents/guardians access to their medical information.
If proxy access is requested for an 11 or 12 year old they must first have a ring back call with their usual GP who will assess Gillick competence. If the clinician agrees that the patient would benefit from the parent/guardian having proxy access online access will compromise of **booking appointments and ordering medication. Proxy access will last until the age of 12 years old or until the patient requests proxy access be removed, whichever is sooner.
In the instance where the patient is deemed competent to manage their own healthcare, an application from should be completed accompanied by photo ID or proof of name and address to be presented in person. A personal email address will be required for this application.
Patients aged 13 to 15 years inclusive
The patients usual GP will be required to assess Gillick competence by requesting a ring back call with the clinician.
If the patient is deemed Gillick competent patients are required to apply for online access in their own right. Photo ID or proof of name and address is required to be presented in person along with a personal email address on the application form. Online access will compromise of **booking appointments and ordering medication.
If a patient is not deemed to be Gillick competent, proxy access can be arranged if the patient agrees to this. Proxy access can be removed at any time at the patients request.
Patients aged 16 years and over
Patients are required to apply for their own online access. Photo ID or proof of name and address is to be presented in person. A personal email address is required for this application.
Proxy access can be agreed with a clinician for patients aged 16 years and over and with a medical condition that deems them unable to manage their own healthcare. This is done on a case by case basis.
Parents/guardians contacting the practice with regard to the results of diagnostic investigations for patients 11 years and over
If our receptionists cannot ascertain from the patients medical record whether the parent/guardian was present at the patients appointment eg blood test appointment, our receptionists will ask to speak to the patient before giving any test results out.
If the patient is not available to speak to our receptionists will ask you to call back when the patient is with you.
If the patient has a medical reason for not being able to speak to our receptionists, please let us know at the time of the call.
